Administrative monetary penalty issued to an individual
On February 2, 2024, the CNSC issued an administrative monetary penalty (AMP) of $10,000 to an individual for the falsification of records related to thyroid screening results.
The individual is a site radiation safety officer with British Columbia’s Provincial Health Services Authority, a licensee authorized by the CNSC to conduct activities associated with diagnostic and therapeutic nuclear medicine.
On July 19, 2023, the licensee reported to the CNSC that an internal audit had identified discrepancies between its paper records and the actual screening tests for thyroid monitoring performed by staff at one of its hospitals in Vancouver, British Columbia.
The licensee conducted a follow-up investigation and discovered that one of its employees had falsified the written records related to thyroid screening for 33 out of 37 required measurements. During the investigation, the individual admitted to falsifying the records to show compliance with internal procedures.
The individual received the maximum penalty amount that can be issued to an individual for this type of violation, considering the violation occurred on at least 33 separate occasions.
